Born in Rome, capital of Italy, Perilli started his career at S.S. Lazio, and then A.S. Roma.[2] He moved to Sassuolo in January 2012.[2] He was a player of their under-17 team.[3] He was a player of their reserve team from 2012 to 2014. On 2 August 2014 Perilli was signed by Lega Pro club Pro Patria in a temporary deal.[4] Perilli replaced Ivano Feola and Giuseppe Messina as one of the first choice keeper.
On 5 July 2015 Perilli received a call-up from Sassuolo.[5] However, on 14 July he joined the pre-season camp of Reggiana instead.[6] The transfer paperwork was completed on 28 July.[7] He signed a 3-year contract.[2] Perilli replaced Feola as first choice keeper again, which the club also released Messina.
On 3 July 2018 Perugia announced that they had signed Perilli on loan from Pordenone with an option to buy.[8]
On 13 August 2019, he signed with Pisa.[9]
On 1 July 2021, he joined Brescia.[10] On 29 July 2022, Perilli's contract with Brescia was terminated by mutual consent.[11]
On 3 August 2022 he joined Serie A side Hellas Verona.[12]
